FHCC Volunteer Nurses

The Free Health Care Center at Catholic Charities could not exist without the generous work done by our community volunteers. Both our dental and medical centers are always welcoming those who would like to volunteer their medical expertise, both hands-on with patients and administrative duties. We also welcome those who are non-clinical and can provide support in the office, events, and special projects.

Care Navigator / Social Work Volunteer

We are seeking a dedicated volunteer trained in social services or case management to serve as the main point-person for patient follow-up. This role is essential in helping patients navigate the healthcare system and access the resources they need.

Key Responsibilities
  • Assist patients with financial assistance applications and charity care programs
  • Schedule specialty referrals, imaging, and preventive services
  • Help with transportation, language needs, and paperwork
  • Track completion of referrals, tests, and next steps
  • Identify and maintain up-to-date community resource lists
Qualifications

This role can be filled by:

  • A licensed social worker, OR
  • A trained volunteer with experience in patient advocacy, case management, or community health
