Care Navigator / Social Work Volunteer
We are seeking a dedicated volunteer trained in social services or case management to serve as the main point-person for patient follow-up. This role is essential in helping patients navigate the healthcare system and access the resources they need.
Key Responsibilities
- Assist patients with financial assistance applications and charity care programs
- Schedule specialty referrals, imaging, and preventive services
- Help with transportation, language needs, and paperwork
- Track completion of referrals, tests, and next steps
- Identify and maintain up-to-date community resource lists
Qualifications
This role can be filled by:
- A licensed social worker, OR
- A trained volunteer with experience in patient advocacy, case management, or community health
