Video and Media Consent and Release Form

1. Permission for Marketing Use
   • By signing below, you consent to allow Catholic Charities of the Diocese of Pittsburgh, Inc. to take and use your photos, videos, and audio recordings.  You agree that we may use these materials for marketing, advertising, and commercial purposes to promote our programs and services.
2. Where We Use Media
   • Social Media Platforms, such as LinkedIn, Facebook, YouTube, Instagram
   • Public Websites
   • Marketing Emails and Communications
   • Printed Brochures and Advertising
3. Ownership and Compensation
   • No payment: You will not receive money or rewards for the use of your media.
   • Full ownership: We own all rights to the photos, videos, and audio recordings we take.
   • No final review: We can edit and use the media without your final approval.
4. Right To Cancel (Revocation Clause)
   • You can change your mind and cancel this consent at any time.  To cancel this agreement, you must send a written notice to us at: Attention Marketing Department, 111 Boulevard of the Allies, Pittsburgh, PA 15222.  Once we receive your notice, we will stop using your media in new marketing projects.  We cannot pull back or delete items that are already printed, published, or shared online.
5. Liability Release
   • I understand that Catholic Charities of the Diocese of Pittsburgh, Inc. may use my photos, videos, and audio recordings as described in this form. I will not hold the agency, its staff, volunteers, or affiliates/partners responsible for issues that may result from the use of these materials.  I also understand that once these materials are shared in print, online, or on social media, the agency cannot control how other people may save, share, copy, or repost them, and I release Catholic Charities of the Diocese of Pittsburgh, Inc. from responsibility for actions taken by others after the materials have been shared.
