Challenges: Options In Aging - Community Services
Individuals receive services in the community and in the home, which may eliminate the need for a move to a nursing home. Income and age requirements may apply.
Care Management
Care Management determines the services you might need through an assessment, recommends services, develops a service plan and provides service coordination and follow-up.
Home Delivered Meals
Frozen meals that meet one-third RDA requirements are provided for disabled individuals or for persons recovering from an illness who are unable to prepare meals.
Personal Care
Personal hygiene and grooming assistance for the frail elderly.
Respite Care
Caretakers of relatives who are age 60 and over, and residing in the same home, can arrange a time-away break.
Personal Assistance
Services are adapted to meet the needs and capabilities of the severely physically disabled adult, to ensure the highest level of independent activity.
Family Caregiver Support
Caretakers of individuals who are 60+ and functionally or care dependant, are provided with caregiver education, benefits counseling and information about financial reimbursement for approved services.
